Managers are required to please complete and return this list by 9th June or prior to first match in preparation for online membership enrolment, medical info & payments, alleviating the money collection by managers.

ALL PLAYERS TRAINING WITH THE AGE GROUP IN THE NEW SEASON MUST BE INCLUDED.

Please return information for each player on this form IT IS IMPORTANT TO ACEPT ONLY CLEAR, FULL AND DETAILS. Surname of both parent & Child are important to be recorded in full & the email address exact. This will save you chasing the information latter & not having that player registered to play.

Should you need, you may include the same information in full, on a separate email to netleyroyal@gmail.com with the email subject line clearly showing the team name for the 2019/20 season e.g. Under 11s Blue This may however take over a week before the player can be entered on the system, hence more efficient for everyone to be done at start of season.
